Lets say something like Internal medicine, GI or Cardiology

Well your overhead will be determined by your amount of space (waiting rooms, # of exam rooms), equipment you want to buy/rent/lease (exam tables, other equipment based on what Procedures you want to do -- scopes, EKG machine, echos, etc). What procedures do you want to do? How do you want to pay for it? Where is it going to be located (big city, small town)? How comfortable are you with technology? Some primary care peeps do laser procedures, or their own scopes, for example.

a minimalist setup might be a primary care location with 1 exam room, all EMR on a single laptop, and refer out for procedures and labs. Tie that in with a little more of a boutique practice, and no support staff (no nurses or admin people) and you could be reasonably profitable if you know what you're doing. I knew someone who had exactly that setup in NY. Quite minimalist, and he could spend more time with patients (they also paid a little extra OOP). He did the vitals himself, everything himself, and really stripped away a lot of administrative BS.

Even this is going to vary based on your location. Your best bet is to find a provider or two who has a practice model similar to what you want to set up, and ask them about it.
