When following the ACC/AHA guidelines about activity levels > 4 METS, a positive YES moves one to surgery on an intermediate risk surgery, whereas a NO moves onto the next level of decision making. The trick is the "unknown". Lots of people tell me that their activity is limited by pain, especially back pain, but that they don't get any chest pain. This is the "unknown" status that needs to get a little more attention. Deconditioning from inactivity might put their heart in a little more precarious position and may warrant more work-up, especially with risk factors.
