Programs Requiring Association of Professors of Dermatology Standardized LOR

I was just wondering if anyone had a list of the dermatology programs that require the Associated Professors of Dermatology standardized letter of recommendation form for residency applicants. I've heard this is a more "hidden requirement" with some programs and I have yet to find it listed as an application requirement on the department websites I've visited. Also, is this form usually required en lieu of, or in addition to a standard LOR from a given letter writer. If anyone can shed some light on this that would be great.
 
I never heard of this, and I am a dermatology resident in the U.S.
 
I had never heard of it either. Last week one of my letter writers asked if I was considering any programs that required this type of letter. He said some programs (such as OSU) had traditionally required it. None of the programs seem to mention it as a requirement, so I'm beginning to doubt its necessity (or perhaps existence). But any insight is certainly appreciated.
