Programs requiring supplemental material?

So I have a list of the programs that ask for a supplemental application/additional docs:
U Arizona
UCSD
UCI
Tulane
LLU
Minnesota
UT San Antonio
George Washington U
Tufts..
Any others? It is kind of late, just making sure that we are not missing anything.:idea:
 
Did U Minnesota ask for supplemental? What did they request? I did not receive the email...

I sent the supplemental for Georgetown U but not George Washington though....
 
U Minnesota asked (by email) to send a one paragraph explanation of interest in the program. George Washington just says that seriously interested applicants should also submit a statement- I guess we are all ''seriously'' interested if we applied to the program, right?

Where did you see a supplement for Georgetown? I was never sent one nor have I found one on the website

I found it on the little blurb about the program when I downloaded the Excel file from SF match:

"Additional non-CAS materials required: CV and Photo must be sent directly to institution. The Georgetown University Hospital/Washington Hospital Center Program in Ophthalmology and the Washington National Eye Center at Washington Hospital Center sponsor a fully-accredited teaching program that provides the eye resident with a comprehensive education in medical and surgical ophthalmology."

Crap! It would have been nice if they sent out an email asking for this, just like most programs! I totally missed this on the excel file.
