Programs that require 4 LOR's

I only have 3 good LOR's and 1 possibly bad one that I don't plan to send out to programs. Anyone know any programs that require a fourth one?

I think so far I only know of Stony Brook and UIC requiring 4.

Actually, according to Dartmouth's education coordinator, they only require 3 LORs per se...apparently, it says four on their webpage, but the fourth is actually your Dean's letter.

UTSW requires 4 I believe.
