Programs that require eye exam

I was wondering if there was any more programs that require an eye exam (acuity, color, and stereo) before they consider your application complete. I only know of U of Kentucky and UTHSCSA. Any one else know of any other programs which need the exam sent to them?

When i applied, i recall many of them did.
