proper way to reject an offer?

What is the proper way to reject an offer?

1) Reply to offer e-mail
2) call person who made the offer?
3) write letter to person who made the offer?
4) A combination of some/all of the above?

Also, what happens if the person who made the offer is different from who you interviewed with and/or who the director of clinical training is? Should all those people be contacted?

joe - write a formal email back to the director of admissions and call the person you personally interviewed with. be cordial and express gratitude for the offer, but don't come off like their program is going down the tubes now that you aren't attending.
 
Avoid e-mail. Most of these people were trained before e-mail, and would appreciate a phone call with a formal, paper letter to f/u. I am 35 and am giving geyser advice........... sigh. 🙁
 
Thanks for the advice; a quick followup. If I call and get a voice mail, should I leave a message about rejecting the offer, or is that better told over the phone ... so I'd just call back when they are in or have them call me back?

No, that would be rude. As was stated before a call and a formal letter is appropriate. You may end up working with some of these people down the road. Best to be very thankful and polite.
