prophylaxis or not?

A 5-year-old girl with ventricular septal defect is scheduled for tonsillectomy in 2 weeks. She has no known drug allergies. Her temperature is 37 C (98.6 F). Examination shows no abnormalities. Which of the following is the most appropriate prophylaxis prior to tonsillectomy?

From new guideline, we do not need prophylaxis, right?
 
AHA 2007
Indications for prophylaxis: only indicated for pts having a warranting condition who are undergoing a warranting procedure

All regimens should be given a single dose 30-60min before procedure


Warranting conditions: prosthetic heart valve, previous infectious endocarditis, cardiac transplant w/ valvuloplasty, UNREPAIRED CYANOTIC CONGENITAL HEART DISEASE, repaired congenital heart defect w/in 6mo or w/ residual defects at site of or adjacent to prosthetic patch/device

warranting procedures: all procedures involving gingival tissue or periapical region manipulation or oral mucosa perforation. Invasive procedures involving respiratory mucosa (incision or biopsy).

No prophylaxis recommended for GI procedures.


Standard regimen is amoxicillin 50mg




I know this is way more thna you asked but as I read your post I realized I didn't know either so I found this off mobile epocrates. This is for PEDS though. Note that it says only cyanotic heart conditions, so you are correct in saying VSD does not require prophylaxis.

The only difference I noticed in the adult tab was that the regimen is amoxicillin 2000mg.
