Hello all, I will be an intern in July and am in the process of completing my pre-employment forms. The forms ask if I've ever been treated or seen a psychologist or psychiatrist. Well I have. Without going into too much I have mild ADHD and some depression which I seek tx for prn. However, I am worried about the implications/stigma of disclosing my history. Should I be? Can this be used against me somehow to deny me employment or otherwise? Please be honest and sincere. Thanks.

Dont worry, go head and say you went to one. If they are not asking why, you are not having to disclosing much. PPl go to a psychiatrist for all different reasons, esp physicians.
 
Be honest. If the answer to one (or more) of the questions is yes, then answer yes and provide a full explanation. Provide documentation to support your explanation. If you have been/are currently seeing a psychologist/psychiatrist, get a letter from that provider that will assure The Powers That Be that you are safe to practice medicine (that's really what the questions are about). Seal the documents (your explanation and supporting documents) in a separate envelope if you want the information to remain private.
 
If they are asking to explain why you had to see one, you will have to produce a letter from your provider.
 
Plenty of people see psychiatrists for non-debilitating problems. They'll probably ask you for an explanation, and your explanation will not throw up any red flags.
