If you're open to the midwest, I would suggest the following:
- Hennepin-Regions (disclaimer, my home program): a strong community-based program with diverse patients/pathology, many different types of training settings and great co-residents/program staff with a family vibe.
- University of Chicago: more academic but small residency classes, great faculty and I hear they are expanding on training sites.
- Indiana: truly under-rated program with academic resources + opportunities for community work
- Didn't interview at but I've heard good things about: U of Iowa, WUSTL.
 
It’s hard to comment on your competitiveness for any particular program, so I’ll just give some names of NYC and Boston programs that bleed rainbow. These are Mt Sinai main (more competitive but the base of a newly founded transgender psychiatry fellowship) and Mt Sinai Beth Israel (less competitive with strong historical ties to the surrounding LGBT community; see Peter Kruger clinic and Dr. Ken Ashley) in New York, and Cambridge Health Alliance (long ties to the local LGBT community) and Beth Israel Deaconess (ties to the close by Fenway Health, which is one of the oldest and most established LGBT community medical care, research and education centers in the country; BI residents can do electives and research projects there as early as PGY-2).
These are only the programs that I know for a fact have strong resources for residents interested in LGBT population. That said, programs in the Northeast are generally supportive of LGBT, so there is a good chance they’ll support your interest even if they don’t already have resources in place.

For women’s health, basically look for programs with a strong CL component that spend a good amount of time on OBGYN. Of the programs I’m somewhat familiar with, these come to mind: Brown (a free standing women’s hospital + clinics), UW (Seattle), BWH and BIDMC. Beware that Mt Sinai Beth Israel in NYC, while strong in LGBT, has weakish CL given that Mt Sinai is moving many medical and surgical services from downtown (where Beth Israel is located), including notably OBGYN. So unless they introduce a new site for their CL rotation, you won’t be getting any experience managing psych issues in pregnant women.
