HELP!

4 of the PT schools I am applying to requires me to provide "Documentation of First-hand Observation and/or work experience related to the practice of PT. This experience must be substantiated in writing by a licensed PT."

Has anyone run into this requirement and what kind of format do I need to use?

I have volunteered for over 100 hours at an outpatient PT clinic over the summer but I did not take notes or have written observations. ES.

Every PT program requires this. You can obtain the official form that needs to be filled out by the PT on your school's website, from your advisor, or from the department office. I would suggest you get in touch with the PTs from the locations you've worked at and bring or fax them a copy/copies of the form so they can fill it out.

Do you mean just a form that records volunteer hours? Not like a big notebook of what modalities you learned each day of volunteering? Thanks! ES.

if the school does not have a form then you need something written on offical letterhead from the facility where you volunteered/observed. it just has to say the dates and total number of hours you were there.
