So in the background section it asks, Were you ever the recipient of any action by any college or university for unacceptable academic performance or conduct violations?

My question is whether I would have to report that I was on academic probation with the honors college of my school. My university GPA was always fine but my GPA of only honors courses was below a 3.0 which put me in academic probation with the honors college.

Good question. I honestly have no idea but you should call PTCAS. Always a good way to go. or ask on their FB page. They answer Q's there.

I was actually unsure if i had violations or not, I mean back in undergrad my roomates and i had a few run-ins with the RA's but I called the schools student relations department, and they said that my student record was destroyed after 5 years but my academic records are kept via transcript/degree etc but any conduct violations would have been destroyed. If you have been out of school a while, your school may do something similar? Hope this helps.
