If one has a manuscript that is published in a pre-print server such as BioRxiv at the time of application (but not yet submitted to a real journal), is it acceptable to list this as a publication? It is unclear at what point in the cycle it will be submitted to a real journal.

Maybe not as a publication, but if I were in your shoes I would include this at the end of the research description on amcas and include the doi # so people can search it + say something like “we intend to submit this work for peer review in the near future”. Pre-print doesn’t mean as much but it’s at least a way for them to type something into a computer and see your name attached to work.
