That's actually a good question.
Outside of the NIH, I don't know a lot of places specializing where there are pulmonary experts specializing in Pulmonary TB, mostly because it's not terribly common in this country - borderline "rare" in people born in the US, and uncommon enough still in even in immigrant populations.
My experience with pulmonary TB is that we tend to help with diagnosis, but that ID (through depts of public health in any given state) drives the treatment.
I'll try and ask this week and see if anyone know where the pulmonary guys are doing a lot of this - I would guess in places where there is 1) a big academic center and 2) a large immigrant population (UCSF or USC come to mind) but I'm not sure.

I went to UCSF for medical school and I recall at least a couple faculty being sincerely interested in TB. San Francisco General sees a fair share of TB so there are opportunities to not only study TB, but also excel in managing it.
I would also look for institutions that have partnered with other nations AND provide global health opportunities for their fellows/faculty, e.g. the Penn Botswana partnership.

San Fran, Miami, Denver, and a place in NY, are the experienced centers in the US, I'm not sure much real research is occurring for TB in the programs. But Denver is also the most competitive academic pulm fellowship out there.
There are programs that you will get lots of hands on with TB, my program runs the county health TB program so I'm very comfortable with everything up to MDR/XDR treatments, as thankfully it's still a rare condition.
