Q on USMLE scoring

i understand that every1 takes diff exams, ie every test has diff # of q's from diff topics and q's are diff - then how can they accurately rank performances ?

Is the bell curve pretty much the same in May then it is in June? (average being somewhere around 216?)
Also, when someone takes an exam A is that curved with all exam As and then given a score? Or does everyones exam regardless of which one go into one big pot?
