Q: thumbnail mirror in 3 mirror gonio lens

a question about the thumbnail mirror in the 3 mirror gonio lens.

we have profs who tell us to put white-out in the area adjacent to the thumbnail mirror, but i think that is plain ugly and gross.

any suggestions on how to mark this area?

Pardon my ignorance, but why would you want to do that?

i'm guessing, but I think it would be to mark the mirror you use for gonio. (to learn)
 
Our instructors made the same suggestion -- it makes it easier to find the bullet mirror (especially if you've already applanated before you try to locate it 😉 ).

You could also use a small dab of nail polish, or use the writing on the lens as a landmark (on my lens, the bullet mirrow happens to be under "ANF+", which is easy to remember -- of course the writing is variable on each lens).

The only other reason I can think of to use white-out or nail polish on your lens would be to distinguish it from your classmates' lenses -- it's pretty easy to lose track of them in lab. 😉

I hate to be blunt about this, but if you need to put white out on your mirror so that you can find it, you probably should not be performing gonioscopy. In my opinion, this applies even to students who are just learning how to do it.

Personally, I always position the mirror superiorly so I know where it is when I put it on. But even when I don't do this, it's easy to find. It's the one shapped like a bullet, not a rectangle or trapezoid. I'm not sure why you need white out or some other aid like that to find it. 😕

how rude are you ?? They're LEARNING. I guess you never put a string on your ret to judge working distance, either. I never marked my gonio either.. but that doesn't mean someone who does it TO LEARN is any less of a doctor.

the profs said to mark it so that the thumbnail mirror is visible to us since many of us dont know what we are doing (ie. pressing too hard on the persons eye, and noting that blood can be seen in schlemms canal)

I'm sorry you feel that way. I was not trying to be rude.

But I stand by my statement. If you need to mark up your gonio lens to just find the mirror, you are probably not ready to start performing gonioscopy.

If you can't differentiate between a bullet, a rectangle and a trapezoid under a slit lamp, that's a problem, IMHO.

Perhaps you are missing the point of marking the lens -- it is so that you can quickly locate the bullet mirror *before* applanation so that it is positioned correctly as you place it onto the eye. I should certainly hope that students are able to distinguish between the shapes of the lenses once they are on the eye!

The reason to mark the lens is that after you've filled it with gonak or celluvisc, it's much more graceful to look for a mark on the side of the lens rather than having to the flip it over and let the celluvisc run down your arm. As cpw said, it's just a learning aid, it's not something you rely on forever.

BTW -- If putting a mark on my lens -- or a string on my ret -- makes it even the *slightest* bit easier for me to learn and master a technique, then I am going to do it. My goal is to become proficient and ultimately expert at what I do, not to be able to say I did it without any learning aids.

the thing is, many students are so nervous when doing gonio, that they end up pressing extremely hard on the person's eye (my partners so far in class). at least with the white-out mark, they'll be more at ease, letting them press more lightly.
 
Hehe. Speaking of gonio, I accidentally backfilled my partner's Schlemm's canal with blood today. I have to learn to relax more. After being in lab since 3 pm today I want to drop kick my gonio lens right now.

Although it is a major thrill when you get a good view.

wow! i've done that almost 5 times already on different people. do you use white-out on your gonio lens like some of us too?
 
also, can someone tell me what i'm looking at when i look into the thumb-nail mirror when doing gonio?

i know where the iris is (closest to the mid area of the lens), but what about the other stuff?

does anyone have a good diagram?

i tried using tape, but it fell off, and now i just succummed to what the profs said, and put WHITE-OUT all over my lens. it's so ugly!
