Page 2 of ucla application asks us to fill out a list of jobs we have held in the last 5 years. they have a table below the question that we need to fill out. the table does not have an option of describing what kind of job i held. it just says give employer name, location, dates, hours and telephone number.
Is that all i should fill out or should i attach an additional page stating nature of my job e.g dental assistant, dentist etc.
Kindly help me out.
