MD Question about AAMC immunization form for VSLO aways

So I was able to have someone at my school sign the form for me after I filled it out.

I am just wondering if I am supposed to attach anything to this?

It says "Copy attached" on the right side of the aamc immunization form for each vaccination so do I need an official proof from the health department or something?

You’re not supposed to fill out that form. You’re supposed to have a physician fill it out. The attachments are for the titers for certain vaccinations. Go to your student health services clinic and have them fill it out for you, they’ll have your titers as well.
