Question about Committee

My committee leader/dental advisor told me I was to put him as a representative/reference on the ADEA application site. However, I do not know where to do that and he takes some time to respond to questions.

I was wondering if anyone here knew what he meant and where to put that info?

What he is talking about is that you put his name as an evaluator on your application.

On your application, go to the evaluators page and put your committee's advisor name as the evaluator and check "committee" letter.

Type in not only his name, but also his email address (or the committee's email address), mailing address, etc.
