How does the FCVS verify credentials from programs, is there a form program directors fill out, or do they just send a letter, or talk with them?

How does the FCVS verify credentials from programs, is there a form program directors fill out, or do they just send a letter, or talk with them?

They send a form that basically says:

Applicant x for FCVS verification provided us with the following information regarding his residency training:

Is this correct ?

Has there been any discplinary action yadayada.


The PD signs it and sends it back.
