Question about Extracurricular/Opto Experience section

Hi,

Does optomCAS just want you to include extracurriculars/opto experience that you had in the 4 years of undergrad? How far do they want you to date things back to? Thank you!

it took me 7 yrs to graduate and then i took another 2 yrs off before appyling, so i put stuff as far back as 2003. i think a good rule of thumb is to include college and beyond, but no high school stuff. other people may have different opinions. i got accepted so it worked for me. hope that helps

Do we have to use the optomcas official transcript request form? I don't live close to school so there's not really a way I can go give them the request form so was just wondering if its mandatory.

You can send transcripts without the form. One set of mine (I went to 2 schools) went in without it and there was no problem. It was actually processed faster than the one with the sheet.

I printed off the school request form from OptomCAS and the request form from my college's website. I put both in an envelope with a check and mailed it to the Registrar at my college.
