So basically, I unknowingly registered for an online course at a different university by accident and ended up failing the class. I've been cleared of the tuition fee, but the F on my transcript is still there (trying to get this expunged). If, for whatever reason, this isn't expunged, would I still need to include the course on my med school application, or can I just exclude it and act like it never happened?
Sorry, but you will still need to send in an official transcript with that F on it and enter it on the transcript within the application. If it is expunged and changed to a W through your appeal for retroactive withdrawal, you will still have to send in an original transcript, but then you can enter a W (or whatever symbol that school uses) on the application transcript (which has no affect on your GPA calculations).

How long will it be before your appeal goes through? Obviously, it would be better to delay your med school application until that happens.
