Hey, I have a question about knee replacements I can't seem to get an answer to on the internet and I was hoping that you guys could help me out. What happens to the ligaments in the knee replacements. I believe the collateral ligaments stay intact but what happens to the cruciate ligaments, and what is present in the knee replacement prosthesis itself to substitute the absence of these parts.

Collaterals remain intact. ACL is removed. depending on implant design either the PCL is retained or removed.
 
Cruciates get sliced and diced and removed. The plastic spacer between the femoral and tibial components has a "stabilizing" cuplike design and takes the place of the cruciates.
 
The collateral ligaments are kept intact. The ACL is taken out and the PCL can be kept or taken out, depending on the surgeon, patient and prosthesis being used. The surgeon will put in "test" implants in the or before he puts in the real ones later during the surgery. The test implants are used to check for fit and for stability and motion of the knee.
 
Just wanted to toss out there that you can do variable releases of the Collaterals if you have a varus/valgus deformity, though in most cases you are not sacraficing the entire ligament.

While we're on the subject, what becomes of the joint capsule?

My understanding is that the prosthesis is lubricated by synovial fluid, but what does the synovium attach to?
