So if you work at a hospital as an emergency doctor (in EM), or as a pediatric neurosurgeon, that's considered private practice?

Private practice can mean different things to different people. To some, it is the ability to influence business decisions regarding things like staffing, billing, etc... To others, it means working in any non-institutional environment. There is no universally defined "private practice." Many universities employ physicians in a clinical setting that is very similar to that of a traditional private practice environement, though they are subject to budgeting decisions made by the university (analogous to the partners of a private practice environment).

For EM, at some hospitals you are on staff but for many hospitals you are a member of a group with which the hospital has a contract. This gives a little more collective bargaining power (but has it's pitfalls as well). It is, however, considered private practice. Many anesthesiologists have a similar set-up.
 
hospital bases physicians frequently are clients of hospitals, even emergency (at some hospitals).
 
I wanted to clear up that by “private practice”, I meant more like a private office that is free-standing and independent from any institution, where you manage operating hours, income, and establish your own clients. I can imagine having a family private practice under these terms would be easier than surgical specialties, such as orthopedics or cardiothoracic. What about others, like ophthalmology or radiology?

I can't. I've been chased off of the Forum In Which the Scared Fire Burns and I dare not speak its true name lest I be discovered, set upon, and banished to the outer darkness of SDN.

Rads is like epitome of private practice man. All of those specialties you listed here have practices that fit what you call private practice. I really don't think any specialty exists that does not, unless your definition precludes one from working in a hospital that they don't own. You can have a practice in the hospital where you establish clients, control hours, and and income but only lease the space/OR from the hospital/healtchare facility.
