I am in a school with a quarter system but will change to semester system. I plan to attand a medical school with a semester system. When that medical school sees my transcript, will they be confused with the quarter system? Also, will my GPA decrase when the school that I am at right now switched from quarter system to semester system?

Many schools have the quarter system, so no, medical schools will not be confused.

Why would your GPA decrease when you switch from quarter to semester? GPA is dependent on the grade you get and how many credits the class is worth. Your GPA might actually increase when you start doing semesters because you will be going at a slower pace than in the quarter system.

When you enter your grades into AMCAS, they will allow you to select if each class was a semester or quarter class and then they will convert them all to semester units. So by the time the schools get them, they won't know the difference.
