Question about re-applying

Quick (hypothetical) question: if I were to decline acceptances or interviews at medical schools this year due to major life circumstances, and then apply to a completely different set of medical schools next year, would the new schools I applied to know where I applied & declined the year before?

I know you have to list if you applied to a school before if you re-apply to that school- but will other schools know somehow that I declined the year before?
 
I'm sure you've already considered and promptly rejected this option, but have you thought about deferring your acceptance? This way you'd have a guaranteed seat for this time next year instead of rolling the dice again. If you did choose to apply again only the schools that you apply to for the second time would see you as a re-applicant. The others would have no idea.
 
Thank you aSagacious, your reply is very helpful. I won't go into the details of the situation, but it's more of a space problem than a time problem, so deferring would not help. I just wanted to confirm that I could confidently apply to other medical schools without having to worry about answering "You were accepted at X, why did you not just go to X if you really want to be a doctor?" sort of questions.

On the primary application it will ask, "have you ever matriculated to a med school?" Matriculation requirements vary by school but range from keeping an acceptance for X-weeks beyond May 15th to registering for classes to showing up for the first day of class. In any case if you haven't accepted any offers then you've got nothing to worry about.
