Question about Reuben Notes?

Where does this document originate from? and is there a place to buy a published version of this pdf? I personally really like the pictures, and colors and want a printed copy if available online... searching the net there is very little info I am able to find about it - or the creator of Reuben Notes.

For those that are unfamiliar with Reuben Notes, its basically a PDF of high yield LEA figures.

A professor from Ohio wrote Reuben, I believe. I could be wrong, but I don't think it's for sale. Most schools have a copy of the PDF floating around.

Started at OCPM for our lower extremity course. He also did a review for neuro too. Reuben is a legend. And as legends go, nobody knows who he/she actually is.
