Never bring attention to a negative.

You can have a LOR writer mention the reason for your poor grades.

Thank you Goro. If there’s a question on another secondary, say one along the lines of “explain any academic inconsistencies” in this case I should explain why here, right?

If you could present yourself to the Committee on Admissions, what would you want to make sure they knew about you?
A used car salesman/woman/person does not make a sale by pointing out all the scratches and dings to the paint and how worn out the tires are but rather by highlighting the excellent condition of the mechanics.

This question is the committee asking you to sell yourself to them - something that you think makes you a value add to the medical profession and their program. You are only given so much space in an application your job is to maximize the positive/interesting things about you and your story, not draw attention to the negative things. Your goal with your written application is to get to an interview. Once in an interview, they can address any concerns about your academic performance and you can be prepared with an answer. Basically, don't give them a reason to extend an interview to someone else.
