After I shadow a number of hours do I require the dentists information, in case they want to validate that I did shadow the number of hours listed.

No one is really going to take the time to ensure that you were honest in listing the amount of hours. Some supplementary applications may ask for more detailed information on the dentist than the AADSAS application asks (e.g., address, phone number, email, etc.) You should keep the dentist's information along with the amount of hours you've spent shadowing.
