Question about UCSF Secondary

Probably a stupid question, but for UCSF's secondary form B it says to check all that apply, yet there's no check mark when you click the boxes. It's just fill in the blank kind of box. The wording is awkward too, "Your Age: 1-5 years, 6-10 years, etc"

Any insights?

BTW, there is not enough space to put "Chem150" etc.. in those boxes... I had to handwrite one or two things and I'm prob gonna get disqualified by those strict guidelines! 🙁 I'm a little worried myself...
