What procedurally oriented programs are the most rigorous, i.e.
-Who does the largest facial repairs?
-in the OR
-under local
-Is anyone moving soft tissue besides cartilage?
-How many rare tumors are seen per week (DFSP, AFX, pagets)
-Who is using Mohs on lentigo maligna/melanoma using
frozen?
slow mohs?

From what I heard...
Zitelli has a large referral center in Pittsburgh, so rare tumors are treated.
Cook does large cases.
 
Mayo Clinic (Rochester) is among the top in the world, given that they have 3 highly regarded senior faculty (Roenigk, Otley and Arpey) as well as two highly productive and skilled junior surgeons who are also emerging as leaders in the field.

The University of Michigan has an unbelievably strong program...next world type of stuff. They have the country's largest number of melanoma and Merkel patients and have an intense volume of cases. 2 yr fellowship, with time for research for those of u who are actually thinking of academics. Awesome place.
 
Most rigorous? Zitelli's practice in Pittsburgh for sure. They do at a minimum 10 cases per day, and they probably do some of the most complicated repairs in the nation. They are also the pioneer of actual Moh's (not slow Moh's) for both noninvasive and invasive melanoma. Very very very few programs in the nation do that.

So there ya go if you are looking for a really rigorous and intensive program. Bear in mind, it's probably not a program for those who also wants a cosmetic procedure kind of experience.
