Hi, some of you nephrologists may be able to answer this Q. When I see patients of advanced age or pts in the process of dying, they quit eating. Their kidneys look ok, no hx of heart failure but they can not tolerate IV fluids, end up needing Lasix. This usually goes on briefly before the family decides pt is hospice. I never got a good answer from our nephron as to what is going on with the patient's physiology. Is it just third spacing due to hypoproteinemia?
