Anyone could explain if plastic fellowship after ortho training refers to hand surgery or plastic in general?

Thanks a lot.

I'm not clear on what you're asking.

While most orthopaedic trained surgeons who do additional training in the field of PRS, do hand fellowships, they are eligible for a full plastics residency after completing their ortho residencies....http://www.sfmatch.org/residency/plastic_surgery/getting_started/requirements.htm

So, while it may be more common to pursue hand after ortho and this may be what you're referring to, you can also do general plastics.
