Question over Background Info

In my freshman year I wrote a paper for my micro-biology class and forgot to cite something and then my professor called me out for plagiarizing the entire paper. In the end i think all i got was basically a warning but should that be included anywhere in the background information?

If it isn't on your transcript then it wasn't significant and hence no one cares for it.
If AADSAS asks if you have ever gotten a warning then put it down.
