question regarding evaluators' information

Does anyone know on the AADSAS application whether, in the field listing evaluators' name and contact information, it is mandatory to list the evaluator's school/institution/business affiliation? I am wondering if there is a asterisk beside that field asking for evaluator's school/institution/business affiliation making it a mandatory field to answer. Thanks.

Is it ok to type N/A or the evaluator's name in the field that asks for evaluator's school/institution/business if the evaluator doesn't want to be contacted at work and list the evaluator's home address in the address field? Thanks.
