Question Regarding to Ordering ADA 2009 Test

So I got the ordering paper (pdf file) from ADA website but there are nor information regarding to international tax and shipping fee.
I am living in Toronto, ON, Canada.
In this case, how much is the shipping fee (ADA standard)??
Also the tax is 13% for Toronto, right?

Also how long does it usually take for ground delivery??

Call the ADA HQ and ask. I'm sure they have info for Canadian shipments.

I found the form to order the package: http://www.ada.org/~/media/ADA/Education and Careers/Files/dat_sample_test_order_form.ashx

It has the shipping and handling information on the bottom of the sheet and shipping to Canada is $30
