I'm filling out the AMCAS app right now, where do I put in the info about me attending the EVMS SMP program? Do I put them as one of the colleges?

Yes. And there should be an option when you enter your information there to show what kind of program it was and I am pretty sure that one of the options is 'post-baccalaureate'.
