on most d.o websites it says that all students have a bachelor's degree does this mean that if you are in your 4th year and have not yet received it but will by the end of the year. basically do students complete it and then apply the next year or do schools know that they will finish it that year?

If you're currently an undergrad, you'll designate your anticipated graduation date on the AACOMAS application. If you're accepted to medical school, it will (obviously) be on the condition that you complete your undergraduate degree. You'll also have to send a final, degree-bearing transcript to the medical school you matriculate at.

You apply before you finish undergrad, but with the understanding that you will graduate with a 4yr degree before actually starting med school. So no, you don't have to wait until after you graduate to apply.
