Here are some questions from Exam II-L (1998-2005). I have the answer key, but I am still a little confused on the reasoning behind some of the answers. (Correct answers are bolded)

Exam II-L (1998-2005)

40. A 34 year old black woman presents for a routine oral exam. She is asymptomatic and there is no evidence of decay or tooth destruction, although slight perio disease is present. Radiographically, there are periapical radiolucencies present on two mandibular central incisors. EPT indicates all teeth are vital (responsive in a similar fashion). Which is the treatment of choice.
A. Initiate RCT on the two teeth
B. Test cavities on the two teeth
C. Identify the dark, potentially necrotic pulp
D. Refer for eval to rule out possible malignancy
E. No treatment needed at this time (This is the answer, but WHY)


48. Which of the following diagnosis represents an injury that causes the tooth to loosen but is not displaced?
A. Concussion --the answer key says this is the right answer, however I thought Concussion was when the tooth was damage but NOT loosened or displaced
B. Luxation
C. Subluxation -- I thought this was when the tooth has been loosened but NOT displaced
D. Extrusion
E. Intrusion
(What are the exact definitions of each, because apparently I have confused some of them)


74. A 22 year old male patient complains about dull pain in the posterior left mandible region. A radiograph made of this area reveals not only a radiolucency around the first molar roots, but also a radiopacity of bone peripheral to this radiolucency. Which of the following best explains this condition? It is
A. normal for this patient
B. A developmental abnormality
C. An error in radiographic techinique
D. A manefestation of systemic disease
E. A reaction to apical inflammatory disease -- Ok, what is this then?


116. A 16 year old patient presents with a firm, non-painful swelling in the mandibular premolar area. Adjacent teeth are vital. Radiographic evaluation reveals a discrete 4 cm RL in the premolar area. Which of the following diagnostic procedures would be preformed next?
A. Excisional biopsy
B. Aspiration -- Ok, why?
C. CT scan
D. Alkaline phosphatase determination

233. 3 carpules (2mL carpules, 40 mg/ml) of local anesthetic X are required to obtain adequate anesthesia. 1 Coz iis a periapical cemental dysplasia and there is no t/t for it.
2 mehicilin is always given parentally.
3 reaction to inflammatory as it is chronic focal sclerosing osteomyitis, not very sure.
233 this ans is wrong as x is more potent.
4 aspiration is done to find out if is blood or anything, to assure for biopsy.
