If one is offered admission to a medical school, with the choice to accept or decline, the choice to accept is not binding, correct? Applicants can hold multiple acceptances until May 15th? Is the option to decline for those with other, more (personally) desirable offers from other schools?

You can hold as many acceptances as you want up to May 15. In other words the choice to accept is not binding.

The option to decline on the form you mail back would be used if you already know that you wouldn't attend the school. I'm guessing this mostly happens because people tend not to withdraw post-interview, pre-decision even if they've already gotten more desirable acceptances.
