Quick LOR questions

I have a form for my LOR and I have to fill out Name, Title, Organization. What is the Title? Is that their position, or title as in MD/PhD?

Title usually means like Professor of XXX or Director of YYY. MD/PhD might have more than one

Yeah, it's basically what Love said. Find out if they're an associate professor, assistant professor, or professor. Use that, e.g. Associate Professor of Biology.
