Pardon my naivete, but does anyone have good references on "demand ischemia" in terms of evaluation, treatment modalities (particularly with regards to whatever literature is available on use of ASA/anticoagulation), and outcomes? The more I read about it through pubmed searches, the more confusing it becomes. (Strange for what should be a simple entity.) I'm seeing cases of patients in critical care units with troponin leaks to the teens and EKG changes, but people are calling it demand and treating with only ASA and beta-blockade. Is there literature on how to identify a troponin leak as demand, and what correlation there is with subsequent cath/stress imaging abnormalities?
