Does a nurse (RN) count as a healthcare provider? Can they fill out the physicals and vaccination forms?
Are you talking about "filling" out the forms or doing the physical? NPs can do the physical.

filling out forms
Great. Someone else looking for a nurse to do the paperwork. 🙄
Cough up the $$ for a secretary. Or better still, do the paperwork yourself. Nurses do have other things to do, you know.
The form says that a healthcare provider must write the form. So am I eligible for that, fab4fan? 😕

1. It sounds like you need a physical for something. In the US generally you have to be a PA, NP, or physician to do a physical (and providers are unlikely to fill it out unless they've done one). If you are in school your student health center can help you out for free/reduced fee.
2. Vaccination form. These can be filled out by PA, NP, physician (RN and Pharmacist in some states). However they have to have access to your vaccination records. If you are like most of us and don't have them the easier way is to get titers done for the different vaccinations.

Vaccination forms, yes. We give vaccines, we can verify that a vaccine was given. Same with TB skin tests--we administer and read them.
For physicals I would ask the place that's requiring you to have a physical. Most likely they require a physician or mid-level, but I know of a few places that are fine with an RN exam.

I'm going to go out on a limb here and guess that you don't work in a clinic.
Because in clinics, no, no they don't.
Not currently, but I did at one time, and I had way too much of my own work to do. I guess it depends on whether or not you're motivated to actually work, though. The work is there, trust me.
Oh boy, an Rn can fill out the form but she is not allowed to sign it. Poor RN's always helping people to do their jobs.
