So my resident on my surg rotation is grilling me with teaching every night.
Anyway, question of the dayy is....why do you have to consider TPN after 7 days of NPO? Why isn't it 14 days, etc.? (i.e. what are you in deficit of after 7 days). I was thinking something alongs the lines of ketoacidosis developing from hypoglycemia...but it's really only a guess.
I can't find a good answer online other than "TPN is recommended to begin 7-10 days after surgery if the patient remains unable to eat."
Thoughts?
