One of the best ways to get a yes answer to “Have you seen bad academic care” is ask a PGY5 rad onc resident.
I know I have and from self appointed “experts”

I don't have a problem with pathways and such. The main issue is enforcement by payors taking away physician discretion. For all the crap I give Potters about his diversity editorial, he is the author of a good pathway type paper on palliative spine treatment. The irony is pathway based medicine reduces any disparity between community and academic centers. Now they are finally realizing their dream with pathways and the like…total control over them.
I've seen some, but in general I'm almost always impressed by how thoughtful most docs in our field are. Even ones in the middle of nowhere.I’ve seen some really bad community care.
We are pretty skewed in residency too. We see the community "f*** -ups" coming in for re-treatment, but often don't see our own "f*** -ups" because those patients don't get followed closely by us.One of the best ways to get a yes answer to “Have you seen bad academic care” is ask a PGY5 rad onc resident.
Ya those academic trained community rad oncs ughh 😏We are pretty skewed in residency too. We see the community "f*** -ups" coming in for re-treatment, but often don't see our own "f*** -ups" because those patients don't get followed closely by us.
No offense to the legendary CRT, but are we going back to this again? Anyone willing to traverse the country on the Oregon Trail and risk cholera with a 21EX on their wagon? I can only carry so much food and ammunition!

Dysentery was the real killer in the game i thought?

Oregon trail probably in sync with "radonc best jobs trail", geographically speakingI think the future radiation oncologist should be able to hunt deer, ford a river, weather storms, withstand infectious diseases, and palliate bone mets, without dying too soon (hopefully, all can retire at 58!). The Oregon Trail and future of radiation oncology is treacherous, Godspeed.

Looks like SERO is living their best life, flush with cash right now!
Drew and Evan "I drink while I contour brain mets" Thomas. Perfect duo .Drew Moghanaki is a terrible person, who happens to have a presence on social media representing radiation oncology. Our field is going through a lot of challenges, and he does nothing but brings an additional negative light to radiation oncology.
He's been discussed here before about his pandering and brown-nosing, but I am glad someone like Brendon Stiles is putting him in his place. Drew needs to step away from the bottle...absolute creep.
I heard about a pretty impressive single iso 40 target plan on a patient with 20 brain mets he was able to generate.Drew and Evan "I drink while I contour brain mets" Thomas. Perfect duo .
What part of the Moghanaki / Styles bit is performative?
I must have missed something… but what I saw of Drew’s argument didn’t seem that unreasonable. What part of the Moghanaki / Styles bit is performative?

We are all human. When we beat each other up for stuff like this, we are only giving society more permission to expect purity from physicians.
If you are going to get personal (and you probably shouldn’t), the most reasonable criticism would involve what one posts on twitter.
Don’t crack jokes on a public forum claiming another physician (by name) allows drinking to impact patient care. It’s almost certainly untrue but, even if it were, it would still be in poor taste.
SDN is great for rad onc because it democratizes the debates that are important to our field by eliminating the threat of punishment for dissension. However, when the focus shifts from debating ideas to attacking individual physicians, SDN is no better than the wall of a bathroom stall.
nevermind the privacy concernsThe drinking comment was on Evan Thomas' own tweet last year:

Sounds like a poll. In all seriousness, his tweet was poster material for anyone trying to suggest we're a country club specialty. I work too damn hard coordinating **** and getting patients treated quicker by weeks than the fractured system in which I work has historically accomplished to be repped on Twitter by that garbage. No s***
His post wasn’t the greatest. Yours was worse.
Edit… I got my posters confused. Apologies
Ah… so your anonymous gossip about another physician drinking is moral because you are concerned about HIPPA. That makes sense.nevermind the privacy concerns
How is his worse ?No s***

Wrong poster. I thought I was replying to @Ray D. AyshunHow is his worse ?
The pixel resolution of modern cell phone cameras is uncomfortably high.nevermind the privacy concerns
Not just that, but an open laptop in a public place. In any case, Lamount is right, I shouldn't pile on, and we've already run him thru the ringer on this. My joke was in poor taste. But worse? Nah.The pixel resolution of modern cell phone cameras is uncomfortably high.
I am strange. My main problem with the post is that I have tried delicate contouring using the touchpad of a laptop. It is about as frustrating as trying to masturbate with salad tongs. And I eat a lot of salad. The drinking comment was on Evan Thomas' own tweet last year:

Better than tossing a lot of it, i guessIt is about as frustrating as trying to masturbate with salad tongs. And I eat a lot of salad.
So it is your belief that, because you work hard, you are compelled to attack a physician who is contouring at ASTRO? … seems like he is working pretty hard as well. I don’t know about you, but I really hate having to contour at a conference, using the s***ty choppy Wi-Fi.Sounds like a poll. Do you really think it is same as working on an aneurism drunk? Im not so sure about that?Sorry. If a surgeon tweeted himself operating while drinking he’d lose his license immediately. Contouring is our version of operating.
Dude played himself. Feel free to pile on, imo.
I once bought a mouse in an airport tech store because I was losing my mind trying to contour with the trackpad. Still have it… solid wired mouse.I am strange. Used to happen decades ago in England/Europe etc... No one made a big deal over a drink probably, but it's just dumb to immortalize that on something like SoMeSorry. If a surgeon tweeted himself operating while drinking he’d lose his license immediately. Contouring is our version of operating.
Dude played himself. Feel free to pile on, imo.
What is your deal? Attacking a physician at ASTRO who is contouring? He was contouring at a table a public bar next to a glass of wine. If you can't see my issues with that, then we really needn't talk about it any more. I don't think you're going to convince me to see things your way.So it is your belief that, because you work hard, you are compelled to attack a physician who is contouring at ASTRO? … seems like he is working pretty hard as well. I don’t know about you, but I really hate having to contour at a conference, using the s***ty choppy Wi-Fi.
Again, attacking people like this is akin to writing cruel things about a classmate, anonymously, on a bathroom wall.
If you feel compelled to slander someone by name, best to sign your own name at the bottom. This way, you will get all due “credit” for your clever observations
It's a bad look any way you slice it. Why take the risk? It's no big deal imo to have A beer or glass of wine while contouring, it just shows incredibly poor judgement to post that stuff publiclySo it is your belief that, because you work hard, you are compelled to attack a physician who is contouring at ASTRO? … seems like he is working pretty hard as well.
