Ransom criteria, assessing mortality risk

• Ransom’s criteria (on admission) : GA LAW
o Upon admission the criteria used include
o glucose >200mg/dL,
o age > 55 years,
o LDH > 350IU/L,
o AST >250IU/dL and
o WBC >16,000/mL.
• Ransom’ Criteria (within 48 hrs following admission):
o Calcium <8
o Decrease in Hct <10%
o PaO2 < 60 mmHg
o BUN increase > 5
o Base Deficit decrease < 4
o Sequestration of Fluid > 6L

Here is what I have in my notes: The mortality risk is 20% with 3-4 signs, 40% with 5-6 signs and 100% with greater than or equal to 7 signs. If the score &#8805; 3, severe pancreatitis likely. If the score < 3, severe pancreatitis is unlikely. So, we count all 11 to calculate the risk?
 
Ranson's Criteria:
GA LAW assessed at admission
C HOBBS assessed within the next 48 hours
Add up all 11 and go from there; the mortality risks you have are correct (thought I've seen 15% risk for 3-4)

Also, the base deficit should be >4, not <4.
