Re-applicant. Nothing much has changed the past year. Pretty much just worked in my lab and no other new extracurricular activities. I did get a co-first authorship in a paper however.

Below is all my stuff:

Asian male senior from four-year public university.
Neurobiology and Psychology double major, with honors in neurobiology.
cGPA: 3.68. (Same as last year)
sGPA: 3.62. (Up 0.2 from last year)
MCAT: 34R

RESEARCH
3 years in neurosurgery lab - Stroke focused research with my main focus on confocal microscopy. Co-first author for paper. (One of my most significant activities)
1 year in psychology lab (9 hrs/wk) - Just random psych research I joined as a sophomore. Analyzed physiological data from kids who underwent psychological tests.

VOLUNTEERING
1.5 years Child Life Volunteer at local children's hospital (3 hrs/wk) - Pretty much hang out with kids during the whole time. Probably the most rewarding and fun volunteering I've ever done. (One of my most significant activities)
4 years feeding the homeless (4 hrs/month) - Went out to a homeless shelter with my church once a month and helped serve food and hand out goods to people of need.
Church summer program Assistant (20 hrs/wk for 8 wks) - Helped with activities, such as art and English, and chaperoned fields trips during summer program for elementary-aged kids.

EMPLOYMENT
1 year Event Services Assistant at student union building (10-15 hrs/wk) - Helped with events ran by organizations ranging from student clubs to school concerts. Assisted in set up, monitoring event, and tear down.
Server/Cook/Busboy at family restaurant (~10 hrs/wk) - Parents just opened up restaurant. Help with everything pretty much. It's actually a pretty fun experience since I love to cook. Don't really get paid though. Just gas and some allowance. Should I list it as employment?

LEADERSHIP
4 years Church counselor for summer and winter retreats (3/4 days at a time) - Worked as a counselor and helped plan the retreats. Also, watched the younger youth and planned and cooked the meals for the retreats.

OTHER/RANDOM
- Co-1st author on paper sent to journal with impact factor of ~10.
- Two time recipient of university's research scholarship ($4000 per scholarship)
- Two poster presentations at university's research symposium
- Honors for neurobiology which included a thesis.

SHADOWING (One of my most significant activities)
- Pediatric Neurosurgeon - 30 hours. Chief of Neurosurgery at children's hospital.
- Pediatric Urologist - 30 hours.
- Orthopedic Surgeon (Oncology specialist) - 20 hours.
- Plastic Surgeon - 30 hours.
- Pediatrician - 25 hours.
TOTAL: ~145 hours

- Science letter from Neuroantomy professor
- Science letter from Neurobiology lab professor
- Non-science letter from Psychology professor
- Research letter from PI (Very strong)

Any advice on what schools to take out? It's currently 38 schools and I'm not sure exactly how many I'll apply to. I've qualified for FAP so the money burden isn't too high and since I'm a re-applicant, I'll have a base of old secondaries that'll help me with my new secondaries.

I'd cut all the Top 25's youre a reapplicant at (You haven't made significant changes to your app - They rejected you the first time and they'll do the same thing again).

I'll might keep around half just because of my publication. Any recommendations on what to take out? I'm more inclined to keep the West coast schools and possibly the NY schools.

Anybody else have some say on my school list? Thanks!
 
Despite your publication, given that is your only major change as a re-applicant with a low GPA/average MCAT for top 25's I'd still suggest you cut them all (Including the UC's).
 
You've got the order wrong. You should list the schools where you already applied. Then list the added schools. Don't focus on ranking. Instead, prioritize the schools according to their "fit", such as location, curriculum, mission, etc.
 
Do you know where you went wrong last cycle? Stats and ECs seem fairly competitive especially for non-top 25 schools.

I did that the first time around, thus the reason why I didn't apply to places like Albany or Creighton. However, I feel I don't have a say in the matter as a reapplicant now. These are all still schools that I had an interest in last year but didn't make the cut, mostly due to location but now I have added them in.

Also, I know rankings arent aren't important but it gives a good parameter of how many per tier I'm applying to.

I don't know. I did apply a little top heavy last year imo, so I've added a lot of mid/low tier schools. Seeing that I only got 3 interviews (1 from in-state) out of 27 schools, maybe my secondaries or PS wasn't the greatest? I AM more confident of my PS this year compared to last year so that might be good. Hopefully I can get some more love this year.

Considering you got 3 interviews last cycle I think what's most important is working on your interviewing skills.
