Hi. Im finishing my residency in Massachussetts. 2 days ago I got my Florida license approved. Started filling out the DEA registration with my FL license # but it tells me I need a state controlled substance license. Im trying to find where can I get the state controlled substance license for Florida but I have found zero/nada. Also, can I put my home address as my bussiness address? I read in another forum that it would be ok.

Any information is more than welcome. thanks in advance.
 
I just found out why. I was putting my home address as my bussiness address which is MA and MA requires a state substance license.
