Reapplying after 5 years with a 9 year old academic IA, do I have an honest shot? (ORM)

To start off, I received an IA my freshman year for sharing homework answers with a classmate. I received a 0 on that assignment and an violation of academic misconduct in my student record. I confirmed with my school that it wasn't considered severe enough to send to other organizations, but I reported it anyway when I applied in 2020-2021. I had a 518 MCAT and GPA of 3.95, applying to 25 schools. I only received 2 II's, one from a T10 and one from my state school, but got waitlisted at both and didn't get off. Looking back, I had a rather unimpressive application, in my opinion I did not have a particularly interesting compelling PS, my EC's were barely adequate, and I wrote all my secondaries in a day or two.

I kind of forgot about medicine after 2021, basically taking two years off working a research technician job to focus on physical health issues. I tried but did not succeed in publishing my project (maybe one day), then moved to a different research job at a medical school. After recovering, I started getting back to my hobbies, interests, and started focusing on my career again. Exposure to the new environment restarted my interested in medicine, and I would like to give it another shot in 2026. Though I haven't really done anything related to medicine in the 2021-2024 interim, I believe I can get at least 500-1000 more service hours, 100 shadowing hours, and 2-3 more pubs by the time I apply. I also have 2000 clinical hours from a year of scribe work, though I will most likely try to find another position on weekends/evenings since it has a been a few years since I had that jobs. I also believe I can score much better than a 518 when I retake the MCAT, but I won't assume as such.

My question is, though, how much will being a reapplicant with an IA hold me back? Would I even be considered at most medical schools, much less T20's?

MO with strong ties to IL, I think I also quality for FAP so I will probably apply very broadly

Report it like you did in your first application attempt, with any updates as recommended or directed by your student conduct office. I concur, you should be fine with this situation otherwise.
