I have read on the applications I have access to that a letter on the doctor's letterhead is a substitute.

Are there any schools that REQUIRE a recommendation form to be filled out?

I know that MSUCOM requires a LOR form-but I'm not sure if there is more.
I think COMP prefers one, but I didn't do it and I got in.

as far as I know VCOM requires the form and u have to have any extra letter writers fill out the 1st page of forms, the waiver part.
